基于寰枢关节不对称与颈性眩晕的相关性研究(1)

     【摘要】 目的:分析基于寰枢关节不对称与颈性眩晕的相关性。方法:随机选择2017年5月-2019年4月于本院治疗的寰枢椎不对称(寰枢椎失稳)患者100例为观察组,选取同期100例健康人为对照组。两组均行三维CT扫描测量寰枢前间隙(ADI)、双侧齿突侧块间隙差值(VBLADI)、寰枢关节齿突侧块间隙(LADI),并进行统计学分析。根据观察组患者的颈部及眩晕症状进行ADI、VBLADI与眩晕症状及各项功能评分相关性分析。结果:两组的左、右LADI比较,差异均无统计学意义(P>0.05);观察组的ADI、VBLADI均显著高于对照组,差异均有统计学意义(P<0.05)。观察组ADI、VBLADI与眩晕、头痛、颈肩痛、日常生活能力、社会适应能力及转颈试验阳性均呈正相关(P<0.05)。结论:寰枢椎不对称患者的ADI、VBLADI和健康人有差异,且与眩晕、头痛、颈肩痛等呈正相关,可作为评估患者颈性眩晕的指标。

    [Abstract] Objective: To analyze the correlation between atlantoepistrophic articulation asymmetry and cervical vertigo. Method: A total of 100 patients with atlantoaxial asymmetry (atlantoaxial instability) treated in our hospital from May 2017 to April 2019 were randomly selected as the observation group, and 100 healthy patients in the same period were selected as the control group. Atlanto-dental interval (ADI), VBLADI, and lateral atlanto-dental interval (LADI) were measured by Three-dimensional CT scanning in both groups, and statistical analysis was performed. Result: Comparison of left and right LADI between the two groups showed no statistically significant differences (P>0.05). ADI and VBLADI in the observation group were significantly higher than those in the control group, the differences were statistically significant (P<0.05). ADI and VBLADI in the observation group were positively correlated with vertigo, headache, neck-shoulder pain, daily life ability, social adaptability, and positive cervical rotation test (P<0.05). Conclusion: ADI and VBLADI in patients with atlantoaxial asymmetry were different from those in healthy subjects, and were positively correlated with vertigo, headache, and neck-shoulder pain, which could be used as an indicator to evaluate cervical vertigo in patients.

    颈性眩晕是由于椎动脉供血不足,血流速度变慢使大脑突然失去血供,从而出现的眩晕,是一种临床常见的多数患者伴有颈部疼痛不适的眩晕综合征[1-3]。颈性眩晕发病机理仍不十分清楚,其病因病机主要有寰枢椎失稳、椎动脉压迫及颈部运动感受器的本体感觉传入错乱等[4-5]。颈性眩晕患者均有不同程度的寰枢椎不对称(失稳),一度认为颈性眩晕与寰枢椎不对称密切相关,并长期以来被认为是寰枢椎关节失稳的一项重要临床症状[6]。目前,直接研究寰樞关节位置关系在颈性眩晕中作用的报道不多见,影像学诊断对寰枢椎关节失稳未有统一标准[7]。因此本文对寰枢关节位置关系与颈性眩晕进行相关性研究,不仅可以为临床诊断颈性眩晕提供影像依据,更可以对枕寰枢椎错位的程度加以量化,从而为评价手法矫正寰枢骨错位的疗效提供客观依据。现报道如下。, 百拇医药(袁娟 林星镇)
