肛周脓肿病原菌分布及对抗菌药物的耐药性分析

【摘要】 目的:鉴定肛周脓肿患者的病原菌分布情况,分析其对常用抗菌药物的耐药性。方法:选取2018年1月-2020年12月吉安市中心人民醫院收治125名肛周脓肿患者,对其送检的肛周脓液进行培养,采用珠海美华MA-120半自动细菌鉴定仪进行细菌鉴定与药敏试验。结果:125例患者的脓液标本中共分离培养出135株菌,其中革兰阴性菌108例,占比80%;革兰阳性菌27株,占比20%;排名前三的细菌分别为大肠埃希菌99株(73.3%)、肺炎克雷伯菌8株(5.9%)和金黄色葡萄球菌7株(5.2%)。大肠埃希菌产ESBL菌株为42.42%(42/99),高于肺炎克雷伯菌12.50%(1/8)。耐甲氧西林金黄色葡萄球菌(MRSA)占比28.6%(2/7),星座链球菌对红霉素和克林霉素的敏感率均为33.3%,对其他常用抗生素的敏感率均为100%。结论:肛周脓肿患者的病原菌分布存在一定差异,病原菌以大肠埃希菌为主。临床用药期间,需要根据细菌培养结果与耐药性分析结果,调整用药方案,避免选药不合理影响药物的治疗效果。
