【摘要】 目的：探讨活动平台单髁置换术(UKA)与全膝关节置换术(TKA)治疗膝关节内侧间室重度骨性关节炎(OA)的临床效果。方法：选取2022年1月—2023年9月上海市第一人民医院蚌埠医院收治的50例膝关节内侧间室重度OA患者为研究对象，根据随机数字表法分为UKA组(n=25)和TKA组(n=25)。比较两组手术时间、切口长度、术中出血量、住院时间、术后膝关节屈曲至90°所需时间、视觉模拟评分法(VAS)评分、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分、美国特种外科医院(HSS)评分、关节活动度(ROM)、关节遗忘评分(FJS)、胫骨近端内侧角(mMPTA)、髋-膝-踝角(HKA)、胫骨平台后倾角(PTS)、股胫角(FTA)。结果：两组均成功完成手术。与TKA组相比，UKA组切口小，术中出血量少，手术时间、住院时间及术后膝关节屈曲至90°所需时间均短，差异均有统计学意义(P<0.05)。术后6个月，UKA组VAS评分、ROM、FJS均优于TKA组，差异均有统计学意义(P<0.05)；两组WOMAC、HSS评分比较，差异均无统计学意义(P>0.05)。术前及术后6个月，两组mMPTA、HKA、PTS、FTA比较，差异均无统计学意义(P>0.05)。两组随访期间均未出现手术相关并发症。结论：对于膝关节内侧间室重度OA，活动平台UKA较TKA手术创伤更小，疼痛缓解更明显，关节功能恢复更佳。
