邻近节段动态稳定系统联合显微镜下MIS-TLIF在腰椎退行性疾病中的应用效果


【摘要】 目的:探讨邻近节段动态稳定系统联合显微镜下微创经椎间孔腰椎椎体间融合术(MIS-TLIF)在腰椎退行性疾病中的临床应用价值。方法:回顾性分析2018年1月—2022年12月于东莞市中医院接受治疗的80例腰椎退行性疾病患者的临床资料,按照治疗方法不同将其分为研究组(45例)与对照组(35例)。对比两组围手术期指标。对比术前、术后3个月、末次随访(术后1年)时两组日本骨科协会(JOA)评分、Oswestry功能障碍指数、椎间隙高度、临床疗效、椎体活动度及视觉模拟评分法(visual analogue scale,VAS)评分。结果:研究组手术时间短于对照组,研究组术后引流量、术中出血量均少于对照组,总有效率(93.33%)高于对照组(71.43%),差异均有统计学意义(Plt;0.05);术后3个月、末次随访时,与对照组相比,研究组椎体活动度、Oswestry功能障碍指数、VAS评分均更低,椎间隙高度、JOA评分均更高,差异均有统计学意义(Plt;0.05)。结论:邻近节段动态稳定系统联合显微镜下MIS-TLIF对腰椎退行性疾病进行治疗的效果较优于单项治疗,手术时间短,术中出血量低、术后引流量少,术后创伤小,有利于缓解患者腰部疼痛情况,改善脊柱功能,利于患者早日恢复正常。
