    [Abstract]Objective： To discuss the efficacy of neuroendoscope-assisted transsylvian insula approach and transtemporal cortex approach in the treatment of basal ganglia intracerebralhemorrhage.Method：A totalof 120 patients with confirmed spontaneous basal ganglia intracerebral hemorrhage in the First Hospital of Hunan University of Traditional Chinese Medicine from January 2022 to June 2024 were selected for retrospective study， and were divided into 60cases in the transsylvian insula group and 60cases in the cortexgroup according to the surgicalaccessroutes.Both groupsunderwent neuroendoscopic surgicaltreatment， withthe transsylvian insula group taking atranssylvian insula approach and the cortex group takingatranstemporal cortex approach.The degree of cerebral hematoma clearance， Glasgow prognostic score before and after surgery，and the occurrence of postoperative complications were compared between the two groups.Result：The residual hematoma volume of the transsylvian insula group after surgery was lowerthanthat ofthe cortex group ......
