B超联合多功能取环钳在吉妮环取环中的应用效果(1)
【摘要】 目的：探析吉妮环取出术中使用B超联合多功能取环钳的应用效果观察。方法：临床选择2016年5月-2017年4月在笔者所在服务站进行吉妮环取出术育龄女性120例，根据取环方式的不同分组，分为观察组(n=60)、对照组(n=60)，观察组进行B超联合多功能取环钳取环法，对照组进行传统止血钳取环法。比较两组的手术时间、术中出血量等临床指标;比较两组成功取环情况;比较两组环尾丝断裂、环铜套散落情况。结果：观察组手术时间、术中出血量等临床指标显著优于对照组，差异有统计学意义(P<0.05);观察组取环成功率高于对照组，差异有统计学意义(P<0.05);观察组环尾丝断裂率、环铜套散落率低于对照组，差异有统计学意义(P<0.05)。结论：育龄女性在吉妮环取出术中使用B超联合多功能取环钳的取环方法可缩短手术时间、提高取环成功率，值得临床推广。

【Abstract】 Objective：To explore the application effect of B ultrasound plus multifunctional clamp in the removal of GeneFiex.Method：A total of 120 women of childbearing age undergoing Gini Ring removal in our station from May 2016 to April 2017 were selected and divided into two groups，60 cases each group.The observation group was performed with B ultrasound plus multifunctional clamp，the control group was performed with conventional hemostatic forceps.The operation time，intraoperative blood loss，successful removal rate，rate of ruptured tail fiber and broken copper sheath was compared.Result：The operation time and intraoperative blood loss in the observation group were better than control group，there were statistical differences(P<0.05).The successful removal rate in the observation group was higher than control group，there was statistical difference(P<0.05).The incidence rate of ruptured tail fiber and broken copper sheath in the observation group was lower than control group，there was statistical difference(P<0.05).Conclusion：The B ultrasound plus multifunctional clamp can shorten the operation time and increase the successful removal rate of GeneFiex.It is worthy of clinical promotion.

我國育龄女性的主要避孕方式之一为放置宫内节育器，使用宫内节育器的数量约占全球的70%～80%[1-2]。吉妮环由于具有无支架、固定式、柔软可随意弯曲、脱落率低等优点为越来越多妇女所接受，但在吉妮环取出手术中应用传统止血钳钳夹环尾丝的方法取环，经常因为环尾丝断裂、环铜套散落情况而需要再使用多功能取环钳或B超联合多功能取环钳的方法才能取环成功[3-4]。有研究发现，育龄女性在吉妮环取出术中进行多功能取环钳与B超联合应用可缩短手术时间、提高取环成功率[5]。本研究选择2016年5月-2017年4月在笔者所在服务站进行吉妮环取出术中予以B超联合多功能取环钳取环，效果满意，现报道如下。
1 资料与方法
1.1 一般资料
临床选择2016年5月-2017年4月在笔者所在服务站进行吉妮环取出术的育龄女性120例，年龄23～40岁，平均(26.7±2.1)岁;孕次1～3次，平均(1.4±0.2)次。纳入标准：符合世界卫生组织制定的宫内节育器取出术的适应证的诊断标准[3];育龄女性;所有患者均为吉妮环取环。排除标准：生理周期不规律者、子宫脱垂或子宫畸形、子宫器质性病变、生殖系统感染、全身性疾病。根据手术方式的不同分组，分为观察组(n=60)、对照组(n=60)。观察组年龄23～40岁，平均(26.8±2.2)岁;, http://www.100md.com(黄夏婷)
