血液分析仪散点图异常对疟疾诊断的应用(1)
【摘要】 目的:分析研究血液分析仪散点图异常对疟疾诊断的应用效果及价值。方法:选择2016年1月-2018年10月在笔者所在城市治疗的18例疟疾患者作为本次研究对象,分析患者的血常规参数以及散点图的变化规律。结果:经血液分析仪分析出三类散点图异常。18例患者血常规基本参数变化中,以PLT降低发生最多。其中5例恶性疟疾患者中血常规指标出现Eo%假性增高1例(20%)、WBC降低1例(20%)、RBC降低1例(20%)、PLT降低2例(40%)、Hb降低1例(20%),13例间日疟换种血常规指标出现Eo%假性增高5例(38.46%)、WBC降低3例(23.08%)、RBC降低7例(53.85%)、PLT降低9例(69.23%)、Hb降低7例(53.85%)。结论:血液分析仪散点图可以为疟疾的诊断提供有价值的信息。

【Abstract】 Objective:To explore the application effect and value of abnormal scatter plot of blood analyzer in diagnosis of malaria.Method:Eighteen malaria patients treated in our city from January 2016 to October 2018 were selected as the subjects of this study.The blood routine parameters and the changes of scatter plot of patients were analyzed.Result:There were three types of abnormal scatter plots by blood analyzer.Among the 18 patients,the decrease of PLT was the most one.Among the 5 patients with falciparum malaria,there were 1 case(20.00%) with false increase of Eo%,1 case(20.00%) with decrease of WBC,1 case(20.00%) with decrease of RBC,2 cases(40.00%) with decrease of PLT and 1 case (20.00%) with decrease of Hb.And there were 5 cases(38.46%) with false increase of Eo%,3 cases(23.08%) with decrease of WBC,7 cases(53.85%) with decrease of RBC,9 cases(69.23%) with decrease of PLT and 7 cases(53.85%) with decrease of Hb in 13 patients with vivax malaria.Conclusion:The scatter plot of hematology analyzer can provide valuable information for the diagnosis of malaria.

世界衛生组织发布的数据显示全球86%的疟疾发生在非洲,9%的病例发生在东南亚,其余5%的病例分布在全球其他地区,我国的云南、贵州、安徽等地为疟疾高发区,随着医学的进步,本地感染的病例已越来越少了,但随着经济贸易的繁荣及援非行动的不断推进,疟疾的发病率又呈现出逐年升高的趋势。该疾病以蚊为传播媒介,会对人类的身体健康产生严重威胁[1-2]。当前临床上广泛运用血液分析仪来检查疟疾,能有效诊断出疟原虫感染,降低疟原虫漏检率[3-4],同时能够减轻医疗人员的诊疗负担。不过在应用血液分析仪检测疟疾时,不容忽视的是血涂片的显微镜检查,主要是因为血涂片显微镜检查复查率降低、血液分析仪散点图异常不够重视等造成,最终容易导致临床误诊漏诊,耽误治疗。本研究对血液分析仪散点图异常对疟疾诊断的价值进行探究,现将结果报告如下。
1 资料与方法
1.1 一般资料
