胃癌根治术后应用早期活动护理对患者胃肠功能指标及预后的影响(1)

     【摘要】 目的：探讨胃癌根治术后患者应用早期活动护理对其胃肠功能及预后的影响。方法：选择2016年1月-2018年5月笔者所在医院行胃癌根治术后住院患者120例，按照随机数字表法将其分为观察组和对照组，各60例。对照组应用常规护理，观察组在此基础上应用早期活动护理。比较两组胃肠功能指标、生活质量及胃乏力发生率。结果：护理后，观察组肛门排气时间、首次进食时间、肠鸣音恢复时间、首次排便时间均优于对照组，差异有统计学意义(P<0.05)。观察组生活质量评分均高于对照组，差异有统计学意义(P<0.05)。观察组胃乏力发生率为3.3%，低于对照组的16.7%，差异有统计学意义(P<0.05)。结论：胃癌根治术后患者应用早期活动护理可改善其胃肠功能及生活质量，降低术后胃乏力发生率。

    Effect of Early Activity Nursing on Gastrointestinal Function Index and Prognosis of Patients after Radical Gastrectomy/ZHANG Boyu. //Chinese and Foreign Medical Research， 2020， 18(15)： -118

    [Abstract] Objective： To explore the effect of early activity nursing on gastrointestinal function and prognosis of patients after radical gastrectomy. Method： From January 2016 to May 2018， 120 inpatients patients after radical gastrectomy in our hospital were selected. They were divided into the observation group and the control group according to the random number table method， with 60 cases in each group. The control group received routine nursing， and the observation group received early activity nursing on this basis. The gastrointestinal function index， quality of life and incidence of gastric asthenia were compared between the two groups. Result： After nursing， the time of anal exhaust， the time of first eating， the time of bowel sound recovery and the time of first defecation in the observation group were better than those in the control group， the differences were statistically significant (P<0.05). The quality of life scores of the observation group were higher than those of the control group， and the differences were statistically significant (P<0.05). The incidence of gastric asthenia in the observation group was 3.3%， which was lower than 16.7% in the control group， and the difference was statistically significant (P<0.05). Conclusion： Early activity nursing for patients after radical gastrectomy can improve gastrointestinal function and quality of life， and reduce the incidence of postoperative gastric asthenia.

    近年來由于饮食结构改变，胃癌患病人群呈年轻化趋向[1-2]。目前我国胃癌早期诊断率较低。临床针对胃癌的治疗首选胃癌根治术[3-4]。术后并发症影响患者胃肠功能的恢复，部分患者甚至出现胃乏力、贫血、感染等术后并发症，极大降低了患者的生活质量。早期活动护理通过对术后患者进行早期的活动锻炼，促进患者胃肠功能的恢复。本研究针对胃癌根治术后患者应用早期活动护理以促进患者预后，现将结果报道如下。(张博宇)
