【摘要】目的：总结闭合性肾损伤合并休克的诊断和治疗。方法：根据外伤史、临床表现、血尿常规、B超和CT检查确定诊断，并明确肾损伤程度和其它脏器损伤情况。先处理直接危及生命的脏器损伤；3例保守治疗；3例行超选择性肾动脉栓塞术；2例开放手术探查。结果：本组病人平均住院34.5天；8例病人痊愈；1例病人入院时已处于濒死状态，抢救无效而死于严重并发伤和失血性休克。随访6例，定期检查血尿常规及B超均正常，且无高血压及肾功能减退。结论：立足于最大程度保护有功能的肾组织，接诊后立即抗休克治疗。休克纠正后，按常规肾损伤处置。在积极非手术治疗后，有开放性手术指征，但短时间内不会危及生命者，采取超选择性肾动脉栓塞术治疗；危及生命或合并其它脏器损伤需手术探查的情况下方施行开放手术，手术时尽量采用保肾方法，降低肾切除率。
