【摘要】目的：术后疼痛是外科康复治疗的难点，是提高患者生活质量，是病人的权利，也是医生的职责，是关系到术后康复的关键，近十年来对术后镇痛有很快的进展，镇痛方法和设备五花八门，效果各异。众多患者镇痛不足的因素是多方面的，而缺乏优良设备，未能形成有效的规范化管理是主要原因。[1]术后镇痛具有减少患者痛苦，改善肺功能，降低术后心肌缺血和高凝状态的发病率，减轻应激反应，提高术后生活质量。方法：选择2000例术后患者，选用北京科联生产的一次性升华牌输注泵(PCA)。规格200ml;药物配方：枸橼酸芬太尼0.1mg×8支，盐酸格拉司琼3mg×1支，0.9%Nacl179ml的混合液，持续静脉泵注，4ml/h,观察术后4.8.20.48小时的镇痛，镇静效果，有无恶心呕吐，皮肤瘙痒，呼吸抑制等并发症的发生。结果：术后48小时观察结果，患者精神状态好，除手术限制活动者外，6小时下床活动1658例，3小时恢复肠蠕动，肛门排气排便，无明显呼吸抑制。镇痛结果：0分1916例，1分38例，2分36例，3分8例，4分2例。结论：静脉术后镇痛确切，止痛效果好，副作用少，无尿潴留发生，术后恢复快等优点，是术后理想的镇痛方法。

    【Abstract】objective: to study the postoperative pain and curative effect of related side effects. Methods: choose 2000 postoperative patients with one-time sublimation brand infusion pump (PCA). Specification 200ml; Medicated: miscible liquids containing with Fantasy Citrate 0.8mg , Granisetron Hydrochloride3mg , and Physiological saline 179ml ,continuous vein pump injecting, 4ml/h, and then to observe the calmness effect of postoperative analgesia 4、8、20、48 hours, and presence of complications such as nausea and vomiting, itching, breathing suppression . Results: after 48 hours observation results, the patients' mental condition is good; excepting for patients with surgical limit activists outside, 1658 cases can get down the bed after 6 hours, restoring anus bowel peristalsis, exhaust, without obvious respiratory inhibition after 3 hours . Analgesia results: 1916 cases with 0 point, 38 cases with 1 point, 36 cases with 2 points and 8 cases with 3 point3, 2 cases with 4 point. Conclusion: intravenous postoperative analgesia has exact analgesic effect, few side-effects, without urinary retention, and recover faster postoperative etc, so it is the ideal demulcent method. ......
