静脉应用胺碘酮治疗80例快速心律失常疗效观察及其对心功能的影响

【摘要】目的 探讨胺碘酮治疗快速心律失常临床疗效及其对心功能的影响。方法 分析1999～2011年住院或门诊治疗的80例快速心律失常患者的临床资料，分析应用胺碘酮治疗快速心律失常的临床疗效及其对心功能的影响。患者发生快速心律失常后首剂静注负荷剂量3～5mg/ kg, 5～10min注入,继之以微量泵维持，如无效15min后可再次予1～1.5 mg/ kg胺碘酮静注, 前6 h按1 mg/ min维持,其后以0.5 mg/ min维持, 24 h总剂量900～1200mg，观察患者治疗前、后脑钠肽(BNP)的变化。结果80例快速心律失常患者中，经静脉应用不同剂量的胺碘酮治疗后,显效58例(72.50%)，有效17例(21.25%)，副作用4(5.00%)。治疗后较治疗前BNP明显降低,差异有统计学意义(p<0.05)，未出现严重不良反应。结论?胺碘酮静脉应用治疗快速心律失常疗效好、作用快，安全无副作用。对患者的心功能无不良影响,值得临床推广和应用。

【Abstract】Objective To investigate the clinical efficacy of amiodarone in the treatment of tachyarrhythmia and its impact on cardiac function. Methods Analysis from 1999 to 2011, inpatient or outpatient treatment for 80 cases of clinical data of patients with tachycardia, tachyarrhythmia analysis of amiodarone therapy on the clinical efficacy and cardiac function. Tachyarrhythmia in patients after the first dose of intravenous loading dose of 3 ~ 5mg / kg, 5 ~ 10min injection, followed by micro pump maintenance, such as invalid again after 15min to 1 ~ 1.5 mg / kg intravenous amiodarone, the first 6 h by 1 mg / min to maintain and subsequently 0.5 mg / min to maintain, 24 h total dose of 900 ~ 1200mg, observed in patients before treatment, brain natriuretic peptide (BNP) changes.Results 80 cases of tachyarrhythmia patients, intravenous application of different doses of amiodarone treatment, effective in 58 cases (72.50%), effective in 17 cases (21.25%), side effects, 4 (5.00%). BNP after treatment than before treatment was significantly lower, the difference was statistically significant (p <0.05), no serious adverse reactions. Conclusions Intravenous amiodarone is effective in treatment of tachyarrhythmia, the role of fast, safe and side effects. On cardiac function in patients with no adverse effects and is worthy of promotion and application.
