    [摘要]5A护理模式由评估(assess)、建议(advice)、达成共识(agree)、帮助(assist)、随访(arrange)五个步骤构成，是知信行模式和延续性护理的结合。在国外已广泛应用于戒烟管理、体重管理、心血管疾病管理、慢病康复管理及癌症管理中，近年来在国内也受到越来越多关注，因此，本文介绍5A护理模式的概念、详述5A护理模式的程序，回顾性总结该模式在国内外的应用现状，分析不足并提出建议，以期为5A护理模式的应用提供理论和实践依据。
