    [摘要]有效的饮食营养管理对延缓慢性肾脏病(CKD)进展，改善预后具有重要意义。CKD 患者饮食营养管理过程包括对患者进行营养风险筛查与评估，制订与落实饮食营养方案，动态评估饮食营养管理效果并适时调整饮食营养管理方案，它是一个长期的、持续的、需要医护患之间密切配合的过程。临床工作中因某些工作人员饮食营养管理理念缺乏，传统的生物医学模式未转变，CKD 患者知-信-行未统一，延续性饮食营养管理干预不足等因素，使饮食营养管理未达预期效果。本文就 CKD 饮食营养管理中存在的问题，从转变工作人员的饮食营养管理理念、适应新的医学模式转变、创新健康教育模式、延续饮食营养管理效果四个方面，对 CKD 饮食营养管理策略总结如下。
