四维超声在胎儿先天性心脏病诊断中的应用价值探究(1)

     【摘要】目的 比较四维和二维超声技术在胎儿先天性心脏病诊断中的应用效果差异,为胎儿先天性心脏病的早期诊断提供借鉴依据。方法 选取2017年10月~2018年10月期间于我院就诊的孕妇76例为研究对象,全部研究对象均给予四维超声和二维超声进行检查,并将检查结果与金标准进行比对,对两种方法的诊断准确率进行观察和比较。结果 四维超声对于胎儿先天性心脏病的诊断准确率为96.05%,显著高于二维超声的69.74%,且差异显著(P<0.05)。结论 采用四维超声对先天性心脏病进行诊断的临床效果较好,与二维超声技术相比四维超声技术的诊断准确率更高,是临床诊断胎儿先天性心脏病的可靠方式之一。

    【Abstract】Objective To compare the differences of the application effects of 4D and 2D ultrasound in the diagnosis of fetal congenital heart disease,and provide reference for the early diagnosis of fetal congenital heart disease.Methods 146 pregnant women who were admitted to our hospital from October 2017 to October 2018 were chosen as the research object in the study.They were divided into the four-dimensional ultrasound group and the two-dimensional ultrasound group according to the different examination methods used by pregnant women,70 and 76 pregnant women in the two groups.The two groups of subjects were examined with four-dimensional ultrasound and two-dimensional ultrasound,and the examination results and the pathological results were compared.The diagnostic accuracy of the two methods were observed and compared.Results The diagnostic accuracy of the congenital heart disease in pregnant women in the four-dimensional ultrasound group was 96.05%,which was significantly higher than that in the two-dimensional ultrasound group of 72.86%,and the differences were significant (P<0.05).Conclusion The clinical effect of four-dimensional ultrasound in the diagnosis of congenital heart disease is better.Compared with two-dimensional ultrasound technology,the diagnostic accuracy of four-dimensional ultrasound technology is higher,which is one of the reliable ways to diagnose fetal congenital heart disease.

    據统计,我国每年出生的儿童人群中患有先天类疾病的残疾儿童的比例约为5%左右,同时胎儿畸形也已经成为导致新生儿临床死亡的重要原因[1]。在这其中,先天性心脏病胎儿人群的构成比例较高,患儿的临床死亡率水平也较高,严重威胁新生儿的生命健康[2]。既往研究认为[3-4]:导致胎儿出现先天性心脏病的诱因较多,其中主要的影响诱因包括丙酮尿症、感染性疾病、免疫溶血性疾病、糖尿病、慢性酒精中毒、不正常妊娠史等。同时,也有一些环境、遗传相关的因素导致胎儿出现先天性心脏病。目前,临床上针对于胎儿先天性心脏病的筛查以超声检查技术为主,传统的二维超声技术在胎儿先天性心脏病的筛查过程中表现出了安全、简便等特点[5]。随着超声技术和相关医疗设备条件的不断改善和进步,四维超声技术在临床实践过程中得到广泛应用。四维超声技术在具有二维超声特点的同时,更够为临床医生提供更加全面的诊疗信息[6]。鉴于此,本研究为比较四维和二维超声技术在胎儿先天性心脏病诊断中的应用效果差异,为胎儿先天性心脏病的早期诊断提供借鉴依据,本文选取2017年10月~2018年10月期间于我院就诊的孕妇76例为研究对象,针对其临床资料进行了比较研究和如下报道。
