替米沙坦不同时间给药治疗非杓型高血压的临床疗效及对患者睡眠质量的影响(1)
摘要 目的：研究替米沙坦不同時间用药治疗非杓型高血压患者的疗效差异及睡眠质量的影响。方法：选取2016年1月至2017年1月上海市宝山区顾村菊泉社区卫生服务中心收治的非杓型高血压患者140例，依据随机数字表法将其分为观察组和对照组，每组70例。观察组接受替米沙坦20：00用药，对照组70例替米沙坦8：00用药，比较2组患者疗效及睡眠质量差异。结果：观察组患者治疗后收缩压、舒张压与对照组患者比较均明显降低，差异有统计学意义(P<0.05)。观察组患者治疗后有58例患者由非杓型高血压转变为杓型高血压，血压类型转变率82.9%，明显高于对照组(62.9%)且存在统计学意义(P<0.05)。治疗前观察组和对照组PSQI评分分别为(7.78±2.34)分、(7.43±2.16)分，治疗后分别为(4.15±1.22)分、(7.36±2.15)分，观察组患者PSQI评分较治疗前明显降低，且显著低于对照组(P<0.01)。结论：替米沙坦20：00用药相较于8：00用药，其对非杓型高血压患者的降血压效果更加显著，同时也提高患者的睡眠质量，更有助非杓型高血压患者转变为杓型高血压，值得临床推广。

Abstract Objective：To study the effect of telmisartan on non-dipper hypertension and sleep quality.Methods：From January 2016 to November 2016，30 patients with cancer chemotherapy after gastrointestinal cancer surgery in our hospital were randomly selected.15 of them were treated with routine care (control group)，15 patients underwent routine nursing and instructed them to perform Ba Duan Jin Fitness (experimental group).The effect of two groups of patients on appetite in the course of postoperative chemotherapy was compared.Results：In the experimental group，the appetite score of the patients during postoperative chemotherapy was significantly higher than that of the control group(P<0.05).In the experimental group，the incidence of other gastrointestinal reactions during chemotherapy (13.3%) was lower than that of the control group (33.3%).There was a significant difference between the groups (χ2=12.657，P<0.05)，the quality of sleep and the time of sleep were better than those of the control group (P<0.05);The PSQI scores of two groups after nursing were significantly lower than before nursing，and the experimental group was lowly than the control group;The average time of sleep after nursing in two groups were significantly shorter than before nursing，and the experimental group was significantly shorter than the control group (P<0.01).Conclusion：The Ba Duan Jin has a positive effect on regulating the spleen and stomach，improving the appetite and improving the quality of life in the patients with postoperative chemotherapy of colon cancer.
