【摘要】收集我院2013年1月～12月2 160例出院31天内非计划再入院患者的资料，回顾性分析患者非计划再入院原因，发现其原因依次为 ：病情复发或反复加重805例，手术并发症381例，疾病知识欠缺353例，出院后未及时随访导致病情加重351例，药物相关问题270例。2014年，我院通过加强对患者病情评估，重视围手术期管理、患者健康教育、医师医疗知识培训及出院后随访等综合质量改进措施，其出院31天内非计划再入院患者例数降至1 589例,与2013年相比明显下降。

    Discussion on the management over unplanned readmission within 31 days after discharge

    【Abstract】The data of 2160 cases of the hospital's patients who are rehospitalized not as planned within 31 days after discharge from Jan. to Dec., 2013 are collected to retrospectively analyze the causes of unplanned readmission, and the causes are found to be relapse or repeated exacerbation of disease (805 cases), surgical complications (381 cases), lack of disease knowledge (353 cases), disease exacerbation due to delayed follow-up after discharge (351 cases), and problems related to medicine (270 cases). In 2014, by taking comprehensive quality improvement measures, such as enhancing the evaluation on patient's disease and attaching importance to the management during perioperative period, patient's health education, medical knowledge training of physicians, and post-discharge follow-up, the hospital reduced the number of patients of unplanned readmission within 31 days after discharge to 1589 cases, realizing a substantial decrease compared with that in 2013. ......
