186例老年心力衰竭的临床特点分析综述(2)
心衰的治疗应积极治疗原发病，去除诱因。用药应针对不同患者的年龄、体重、一般情况、肝肾功能等从小剂量开始，密切观察病情变化，监测肝肾功电解质，随时调整用药剂量。常用药物包括利尿剂、ACEI制剂或ARB制剂以减轻心脏前后负荷，改善心肌重塑；洋地黄制剂增强心肌收缩力。心功能改善后如无禁忌如房室传导阻滞、窦缓、哮喘、血压过低等应尽早使用β—受体阻滞剂。本组资料显示，经过积极治疗原发病和去除诱因治疗，绝大部分病人均好转出院，死亡率低。
老年心力衰竭为临床常见病，虽然不可治愈但可防治。对已出现功能不全的患者应积极预防诱因以减少急性心衰发生。但有冠心病、高血压、风心病、扩心病、肺心病等病的患者应积极治疗原发病，延长出现心衰的时间，提高患者的生存质量。
